To keep everything running smoothly and safely, I ask all new and existing clients to complete a brief registration form before scheduling services. This helps me understand your pet’s needs, confirm which services you’re interested in, and make sure I have the right consents in place.

• Based on your selections, you may see a few additional consent sections

• Everything can be completed and signed electronically — no printing required
